Will your players find themselves ambushed by a Blood Bot bursting through a nearby wall; awakened in a Vermillion Order healer’s tent with a secret laboratory beneath it; trapped before a pulsing Blood Sack blocking a dark tunnel, locked inside a moonlit conservatory with a ravenous Bloodfly, or defending a sickly man as Wither Commoners crawl through the windows of his apartment at night? In Calydon, even the healers have secrets, and every darkened window, alley, and sewer tunnel might be hiding something hungry.

This supplement includes 5 new statblocks and our latest monster template from the blood-stained streets: battle the Blood Bot, a blood-fueled construct of glass and metal that bursts through walls and drains victims with a massive needle; flee the Blooderfly, a huge blood-drinking insect that grows more violent the longer it feeds; face against the Hemoscoma, a corpse-wearing caterpillar-like horror that armors itself in the bodies of its victims before undergoing a grotesque metamorphosis; beware the Vermillion Order Healer, a false medic who hides blood magic, disease, and experimentation behind a mask of mercy. And pray the Withering Death does not reach your table, as ordinary creatures turn into diseased Undead that crawl through windows, drain life force, and spread wasting death through the city.

Player Options

You also get our latest Gravebound player subclass with the Mellitumancy Devotion subclass. Mellitumancers study the Withering Death, a bloodborne disease that starves the body while drowning it in excess. They have learned how to weaponize their own infected blood, spreading thirst that cannot be quenched, hunger that cannot be satisfied, and numbness that crawls through the limbs like a slow death. Their soul’s companions carry the Withering into battle as undead vessels of contagion, hunger, and wasting decay. Mellitumancers see blood as the perfect catalyst for imbalance, infection, and the slow collapse of the body from within.

This supplement also brings a new mouseling subrace, the Hemascul. Born from the cruel experiments of the Vermillion Order, hemasculs are blood-touched survivors shaped by surgical study, controlled infection, and blood magic experimentation. These resilient mouselings can smell blood, sickness, injury, and disease, track wounded creatures through the city, and even sacrifice their own vitality to save others.
